Investigation Summary
At 6:00 p.m., Employee #1and coworkers started work on a reconstruction project of the West Grand Boulevard overpass. They put up barricades as per the Michigan Department of Transportation's (MDOT) traffic control plan, closing down one lane at 7:00 p.m. and another at 8:00 p.m. Working from an elevated work truck, the crew began stripping forms from the underside of the bridge. Employee #1 was cleaning the lumber on the ground. MDOT inspected the closure at 9:00 p.m. and again at 12:30 a.m. The foreman was concerned about the speed of passing vehicles and called the State Police, but did not receive a response. A car, coming from an unidentified direction, sped down the gutter lane and struck Employee #1 and then hit two pickups parked nearby. Employee #1 was taken to Detroit Receiving Hospital, where he died. The employer was cited for a violation of T8CCR 408.42223(2).
