Employee crushed by sky trak

An operator of a Sky Trak 6036 instructed an employee to hook a lifting strap that was hung upon the wall to the forks of the sky trak. The employee grabbed the strap and swung toward the sky trak. His weight tipped sky trak, and it fell onto him, crushing and killing him.
