Employee dies when crushed between trailer tongue and ground

On January 10, 1995, Employee #1, of First Class Homes, was helping a coworker place a jack stand under the tongue of a trailer. The tongue slipped off the temporary support and crushed the employee's head between the trailer tongue and the ground. A citation was issued for a serious violation of 4081-0034(12) and OSH 4082-1111(1) /1117(1).
