EMPLOYEE TRAPPED BY FALLING ELEVATOR CAR

Employee #1 was lying on the floor with half of his body extended out over an elevator shaft. The elevator car was situated only a few feet overhead. He activated the elevator operation switch in the shaft and the elevator descended. Employee #1's upper body became trapped under the car and in the shaft. All of the safety devices had been bypassed, and the operation controls were not locked out.
