Employee killed in fall through roof opening

On November 18, 1996, Employee #1 and two coworkers were installing a roof vent on a fully decked roof. One coworker was cutting out the hole, and Employee #1 was doing the measurements and helping with installation. The coworker cut one long side and one short side, and then left the roof to replace the saw blade. While he was gone, Employee #1 decided to transfer more measurements around the opening. He had finished measuring when he inadvertently stepped back into the opening and fell through to the next level. Employee #1 was taken to Detroit Receiving Hospital, where he was pronounced dead.
