Investigation Summary
At 11:45 a.m. on July 11, 2017, Employee #1 had just completed joining two rolls of materials and reactivated the machine. The employee then attempted to clean the build up of latex off the idler roller while the machine was running. Employee #1's arm was drawn in between the two idler rollers, causing crushing injuries that resulted in a broken forearm. The employee was hospitalized for two or more days.
