Investigation Summary
On October 5, 1995, a crew of bricklayers and laborers that included Employee #1 was working at the commons building from a scaffold approximately 98 ft long and 26 ft high. Employee #1 ran out of mortar and decided to take his break. He went to the ladder but when he stepped on it, the ladder slid down. Employee #1 lost his balance and fell to the ground, sustaining multiple fractures. He was transported to Mecosta County General Hospital, where he died the next day.
