EMPLOYEE CRUSHED BY FALLING BRAKE MECHANISM

Employee #1 was standing on a ladder installing a suspended brake mechanism on a power press. One leg of the rigging broke away from the brake housing, knocking Employee #1 off the ladder. The weight of the brake mechanism then caused the trolley to break, which allowed the brake mechanism to fall onto Employee #1. He was struck by the heavy mechanism and killed.
