Investigation Summary
Employee #1 ripped a 2 in. by 6 in. (nominal) by 18 in. piece of lumber on a 12 in. table saw with the blade raised to the maximum height and the guard in the raised position. After completing the cut, he reached near the saw, presumably to remove a scrap piece approximately 1 in. wide. His hand contacted the exposed blade, amputating all five fingers of his left hand.
