Investigation Summary
Employee #1, a leased employee, was applying or carrying a 4 by 8 ft sheet of plywood across the roof of a two-story residence in Portland from a stack he had handed up from the second story. He slipped or tripped and slid from about 4 ft above a "California Corner" style toeboard, wherein a 2 by 6 on edge is nailed to a 2 by 4 nailed flat on the roof about 2 ft above the eave. Employee #1 bounced over the toeboard and fell about 22 ft to uncompacted, muddy earth, breaking his back in four places. He was transported by ground ambulance to St. Vincent's Hospital and from there was sent to OHSU's trauma center.
