Employee injured when struck by forklift

At approximately 1:40 p.m. on December 9, 1993, Employee #1 received a fractured left wrist as a result of being knocked down by a 5,000 pound capacity forklift. Evidence indicates that Employee #1 hastily exited her foreman's office and did not look in either direction before entering a lane used by forklifts. The lift driver saw her exit the office and braked to a near stop but was unable to avoid a collision. A cardboard box on the forks struck Employee #1 and her wrist was broken as she attempted to break her fall to the concrete floor. An effective means of isolating pedestrians from forklift traffic was not provided.
