Investigation Summary

Investigation Nr: 676726
Event: 11/02/1993
Employee catches arm in in-running conveyor nip point

Employee #1 was employed as a part-time utility worker. At the time of the accident he was feeding raw material, consisting of ground-up rubber, onto a feed conveyor with a shovel. A small piece of rubber had fallen beneath the conveyor belt and had become lodged on the tail pulley where it rotated with each revolution of the pulley. Employee #1 attempted to dislodge the debris by poking it with a two-foot long piece of solid conduit that he found lying nearby, without turning off the conveyor. The conduit became caught in the in-running nip point. Instead of releasing his grip on the conduit, Employee #1 attempted to pull it out, but was pulled into the nip point up to his shoulder. After a short time another employee finally heard the cries for help and shut down the conveyor and released Employee #1. Employee #1 was immediately taken to the hospital where he was treated for a crushed and fractured arm and released after a day and a half. Contributing factors to the accident were the unguarded in-running nip point, the lack of conveyor controls near the operator's station, and the relative ease of finding the length of conduit apparently left from the recent installation of the conveyor.
