Investigation Summary
At 2:30 p.m. on October 3, 1994, Employee #1 was atop a three-story structure helping a coworker slide some trusses around to get them ready for rolling. Employee #1 walked up the stairs to where the trusses were located and allegedly tied a rope around his waist. While pulling on the trusses, he slipped and fell over the edge of the roof, falling approximately 28 ft to the soft earth. Employee #1 was hospitalized with a fractured wrist and sore ankle.
