Investigation Summary

Investigation Nr: 170804280
Event: 04/17/1995
Employee injures back in fall from roof

Employee #1 was unloading composition shingles onto a 7 1/2:12 pitched roof. Approximately 70 to 80 bundles were already on top when another employee arrived to help. Employee #1 unfastened his safety lanyard to allow the other worker to come up on the roof and take over because he was going to another level to install some boards. On his way down to the ladder, Employee #1 noticed some bundles that were not square. When Employee #1 attempted to straighten them, he slipped and fell approximately 25 ft onto muddy ground. He was hospitalized with back injuries.
