Investigation Summary
On October 19, 1994, Employee #1 was cutting wedges from 2 by 4s on a table saw. He removed the blade guard so the material being cut would not hang up on the guard. After completing a cut, he twisted the wood to pick it up. The wood was grabbed by the blade, jerking his hand into the teeth. The little and ring fingers on his right hand were amputated.
