Employee's arm crushed in banding machine

Employee #1 had been recently hired to work on a metal banding machine. While the bander was cycling, he attempted to replace a protective cover on some stock that was being banded for shipment. His left arm became caught in the banding and was crushed. Employee #1 had been given about two hours of training and was not shown any type of policy or procedure on how to safely run this machine.
