Investigation Summary
On December 12, 1996, Employee #1, a carpenter, was on a forklift work platform installing plywood fascia at the eave line. He lost power to his circular saw and he was unable to get the forklift operator's attention. Employee #1 then decided to access the 4:12 pitched roof and walk along the peak to an opening 80 feet away, where he could enter the building and check the power. He slipped on the frosty plywood sheeting near the peak, slid approximately 25 feet, and fell three stories to muddy ground. He was hospitalized with a fractured pelvis and tailbone.
