Employee's lower leg crushed by falling pipe

Employee #1 was removing the straps from a load of 16 inch HDP gas pipe to unload a flatbed truck. The pipes were approximately 40 feet long and weighed more that 1,400 pounds each. They are normally banded together in groups of three, but two pipes on the seventh level were not banded. When Employee #1 released the straps, one of the pipes fell and crushed his left ankle and foot. His leg had to be amputated below the knee.
