Investigation Summary
At approximately 7:15 a.m. on June 5, 2017, Employee #1 was watching a forklift operator lift a steel table. He decided to help by placing a 4 in. by 4 in. wood block under the steel table. The steel table fell off the fork of the forklift, striking and crushing one of his fingers. Employee #1 was transported to a medical center, where he received treatment on the crushed finger, including amputation of the finger and was then released from the hospital.
