Investigation Summary
At approximately 9:30 a.m. on June 23, 2017, Employee #1 was using an 8 ft Werner stepladder to tuck a wire into a cable tray within an electrical control room of an electrical substation. After tucking in the wire, Employee #1 descended the ladder. When he stepped to the floor from the ladder' s second rung, he lost his balance and fell to the floor, falling approximately 1.5 feet. Employee #1 suffered a right hip fracture in the fall. Employee #1 was transported to a medical center, where he received treatment and was then hospitalized.
