Investigation Summary

Investigation Nr: 201630431
Event: 02/10/1998
EMPLOYEE INJURED WHEN RUN OVER BY LOADER

At 3:30 p.m. on February 10, 1998, Employee #1, the owner of Pacific Forest Contractors Inc., was in the process of walking from the log deck to talk to the loader operator who was working. Employee #1 walked up to the right side of the loader which was next to a steep bank. When he had reached the front of the loader track, his toe caught on debris and he fell down in front of the right track of the loader. At this same time the loader operator started moving forward, running the loader's right track up the left leg of Employee #1. Employee #1 yelled, getting the attention of the operator, who immediately stopped the machine. Seeing what had happened, the operator backed the track off of Employee #1 and called an ambulance which took him to Mount Hood Medical Hospital. Employee #1 received a broken ankle and leg lacerations, and was hospitalized.
