Investigation Summary
At 9:29 a.m. on July 4, 2017, an employee was writing a receipt date information onto cartons of food items that was placed on a storage shelf inside the walk-in cooler. The employee fell three feet from a step ladder when a box or boxes fell from the shelf toward her face. The employee was transported to the hospital where she was examined.
