Investigation Summary
At 1:00 a.m. on August 11, 1996, at a mint still in Jefferson, OR, Employee #1 crawled under a farm truck parked on a slight incline. The truck began to roll slowly backward down the incline, catching Employee #1 behind the head and bending him in half at the waist. As the truck rolled over him, Employee #1 suffered compression injuries from the front axle and steering bar.
