Investigation Summary
At approximately 12:00 p.m. on July 13, 2017, Employee #1, a delivery truck driver, was working for a masonry and landscape supply company. He was delivering landscaping materials on a forklift near an intersection. Employee #1's flatbed delivery truck was parked on the westbound center turn lane of a road, which he was unloading material with the forklifts. Employee #1 did not use any traffic control devices to warn vehicular traffic. He had just completed his delivery and was on the forklift to the right of the flatbed truck in the middle of the westbound lane of the road. A driver of a personally operated vehicle (POV) was traveling westbound on the same road at approximately 45 mph and suddenly saw Employee #1's delivery truck and forklift. The driver slammed on his brakes. Employee #1 heard, then saw the vehicle skidding toward his forklift and he leaped from the forklift. The vehicle struck the forklift, which was then pushed into Employee #1's left ankle and fractured the left distal tibia. Employee #1 was transported to a medical center, where he received treatment on the ankle fracture and was then hospitalized.
