Employee amputates fingertips in nip point

Employee #1 had set up a tenter frame and was attempting to run paint roller cloth. He started up the machine, but the cloth did not align properly. Employee #1 tried to align the cloth by standing on a bucket and reaching over and behind the guard while the machine was running. When he tried to align the cloth between the nip point at the wire brush and selvage pins, he caught his fingers in the nip point. The middle two fingertips were amputated and Employee #1 required hospitalization. The company has a lockout program and a procedure for aligning material, and Employee #1 had undergone comprehensive training. The cloth moves at a maximum speed of 45 feet per minute. The machine is equipped with emergency stops on both sides for its entire length.
