Employee suffers fingertip amputation in power press

Employee #1 had his hand in the point of operation of a Chicago 500 ton part revolution mechanical power press, which was in "inch" mode. The press has two sets of palm buttons and can be activated by one set if the press is in the "inch" mode. When a second operator activated the press Employee #1's right index finger was amputated to the first joint.
