Employee killed in fall from forklift

Employee #1 was performing his normal job duties pulling tires and logging tire sheets. A forklift raised him 15 ft high on an unsecured pallet with no guardrails. He was not tied off. He slipped and fell to the concrete floor, sustaining shattered heels, a cracked pelvis, and a fractured skull. He was killed.
