Employee injured when caught in cooling bed

On December 12, 1994, Employee #1 was conducting his regular duties as a contract repairman at a steel production plant. He was working with a coworker to investigate a hydraulic leak to the RM #1 cooling bed. Employee #1 went along the top of the cooling bed while the coworker went along the side, assuming that Employee #1 was following him. While on top of the cooling bed, Employee #1, without instructions to do so, removed a grating and attempted to take a short cut to the coworker's location by dropping through the cooling bed. A bar arranger then pinned Employee #1 to the cooling bed frame. Employee #1 received broken ribs, a broken arm, and a cracked pelvis.
