Employee struck and killed by wire from machine

Employee #1 was operating a Lewis wire cutting and straightening machine that he had just finished lubricating. The tightly wound wire stock had come to the end of the coil on the spool (where wire is fed into the machine). As the end of that wire came off the spool it was under tension. It whipped around and struck Employee #1 in the head, puncturing the skin in his temporal lobe area. He died approximately 7 hours later. The cause of death was reported to be a subarachnoid hemorrhage due to an aneurysm. The gauge of the wire Employee #1 was using that day measured .580 on a 30-inch diameter spindle.
