Investigation Summary
Employee #1 was operating an EB3 machine in a tire manufacturing plant. The 1.8 horsepower machine rolls rubber through calender rolls from a conveyor at a speed of 150 rpm. Employee #1 was at the rolling end of the machine, which had begun to roll out of line. He was attempting to realign the rubber by touching the skim rubber when his hand was caught in the pinch point and pulled in. Employee #1 sustained a broken forearm and his pinkie finger was later amputated.
