Investigation Summary
At 8:30 a.m. on March 9, 1995, Employee #1, of Data Tele Enterprises, was using a JLG CM2033 electric scissor lift to install communication cable in the 25 ft high ceiling of an Eagle hardware store under construction in Orem, UT. He had extended the lift to reach the ceiling and as he was moving it, one of the wheels hit a depression in the floor where a sewer clean-out had been placed. The wheel of the scissor lift dropped 2 1/2 in., causing it to tip over. Employee #1 fell with the lift and struck the floor, sustaining head injuries and a possible leg fracture.
