Employee killed in fall from ladder

On February 10, 1995, Employee #1 and a coworker were installing plywood decking on the second floor of a two-story residence. Employee #1 descended an aluminum ladder to obtain more nails from the first floor (garage) and started back up. The ladder, which lacked safety feet, slipped on the smooth concrete and Employee #1 fell with it, striking his head on the concrete floor. When the coworker, who had heard the ladder fall, could not get a response from Employee #1, he called 911 (the call came in at 10:11 a.m.). The paramedics responded and called in Life Flight. Employee #1 was pronounced dead at the hospital. The maximum distance Employee #1 could have fallen was 11 ft 3 in. There were no witnesses to the accident.
