Employee's hand/wrist caught and crushed in press

Employee #1 was adjusting an A-line igniter tube press with the press tooling. He had the press in the manual mode so that the air actuated press cylinder could be raised. To actuate the cylinder, he had placed an Allen wrench into a voltage override opening located on the lower right side of the back of the press at knee level. When Employee #1 reached into the press area to retrieve a part, he accidentally bumped against the Allen wrench, causing the press to activate and the cylinder to rise and complete the cycle. His left hand and wrist were caught and crushed. The accident's causal factor was improper use of lockout/tagout procedures.
