Employee injured in fall from extension ladder

Employees were on two 30 ft extension ladders located on opposite ends of a scaffold, adjusting an advertising sign in the Delta Center. The bottom of the ladder from which Employee #1 was working slipped on the concrete surface, causing him to fall 24 ft and shatter both ankles. The ladder was not tied off and the bottom was not braced to prevent movement.
