Employee injured in fall from unsecured ladder

Employee #1 was untying a 12 ft long, job-made wooden ladder at the top so he could move it to another location. The ladder, which was resting on a snow- and ice-covered surface, did not have slip resistant feet and was not secured at the bottom to prevent movement. Employee #1 stepped onto the second rung from the top of the ladder and fell approximately 8 ft to the surface. He suffered a fractured skull and upper left arm.
