Employee's legs crushed by falling beam

Employee #1, a lift operator, was setting a beam in a saddle that was designed to lift beams in the interior of a building. A burr caught on the edge of the saddle, causing it to fall. Employee #1 sustained crushing injuries to both legs when the beam fell on them.
