Investigation Summary
A truck driver and his assistant arrived at a site and parked their log truck and trailer loaded with 30 ft logs. Because it was late, they decided to unload the truck in the morning. Employee #1, the site manager, pulled the truck into the yard and began to unload it himself at twilight. He undid the chains that had secured the load during travel, then, using a pry bar and a small sledgehammer, began to reach under the load to release the cabling that would release the stakes and the logs. Employee #1 was at the cab end of the truck beside the load (opposite from where the logs would roll off). When he released the cabling, two logs rolled off the wrong side and struck him in the head and chest. Employee #1 was killed. This kind of cabling mechanism is standard in the logging industry. The method of dropping the stakes is an accepted practice and is used in the industry. Some companies use a sling method because they feel the procedure of releasing the stakes is dangerous.
