Investigation Summary
Employee #1 was making a wind brace piece on an unguarded 25 ton Bliss press. He had been assigned the task before and was trained in the use of the press. Employee #1 reached into the press to remove the piece that he had just made. While he was reaching in, the press cycled, bottomed out, and crushed four fingers of his right hand. Another employee heard him yell and came to his rescue. They reversed the machine and got his hand out of the press. Employee #1 was then taken to the hospital. The fingers were amputated past the first knuckle.
