Investigation Summary
Employee #1 was working on the new Jordan Middle School early in the morning, placing masonry blocks on the wall of the gymnasium from a scaffold 14 ft above the ground. It had rained the day and night before and the scaffolding was very slippery. He lost his footing (slipped), slid under the guardrail, and fell 14 feet to the ground. Employee #1 was hospitalized with head and neck injuries.
