Employee loses vision in eye when struck by grinding stone

Employee #1 was in the shop, grinding on a new casting with a pneumatic hand grinder when the grinding wheel flew apart. A piece of the stone struck him under the left eye and then traveled up at an angle under his nose, causing damage to his right eye and his eventual loss of vision in that eye. The guards had been removed from the grinder.
