Investigation Summary
At 2:00 p.m. on August 4, 1993, Employee #1 was helping to move precast concrete from a storage area to a flatbed truck. The employees were using a forklift with a hook and wire rope in a bridle arrangement to hook onto the ends of the precast concrete (I-bolts are embedded in the concrete for the hooks). The forklift would raise the concrete and transport it to a flatbed truck where it would be lowered and unhooked. Employee #1 was riding on the forklift on the other side of the driver when he slipped/fell/jumped from the moving forklift. His leg was run over by the rear wheel.
