Employee injured in fall onto subway tracks, later dies

On April 1, 1999, Employee #1, of the New York City Transit Authority (NYCTA), fell onto the track bed at a subway stop in Queens, NY. He was transported to Mary Immaculate Hospital and admitted for treatment. On April 8, 1999, the NYCTA System Safety Office telephoned the New York State Department of Labor to report that Employee #1 had died in the hospital.
