Investigation Summary
At 11:15 a.m. on July 5th 2017, an employee was approximately 17 feet up on a 24 foot Werner extension ladder painting a window frame. The ladder became displaced the employee fell and suffered a head trauma. He was discovered by the homeowner after she reportedly heard the ladder fall and went out to investigate. Emergency services were contacted and, upon arrival, determine the employee to be dead approximately 15 minutes later.
