Investigation Summary
At approximately 11:30 a.m. on June 27, 2017, four employees were pouring a slurry concrete at a rebar wall against the slope. The scaffold gave way, all four employees were injured when scaffold fell and the rebar wall fell on top of the employees. The employees suffered from multiple fractures resulting in hospitalization of two employees.
