Employee crushed to death by transfer unit

On April 26, 1995, Employee #1, of LTV Steel Tubular Products, a 36-year-old male, was attempting to straighten out tubes on the exit staging arms of an automatic cleaning line. The automatic rail car parts transfer unit moved into position and caught and crushed his neck and back. The unit was not locked out. Employee #1 died. A citation was issued for serious violations of 11(3), 21(4), 147(c,4,1).
