Investigation Summary
At approximately 3:30 p.m. on June 12, 2017, Employee #1 was cutting and removing damaged from porcelain tile on the ground. The tiles had been offloaded in an A frame crate from a flatbed truck, onto the sidewalk adjacent to the site. Each porcelain tile weighed approximately 200 pounds and had dimensions of 5 ft by 10 ft. The remaining undamaged tiles were placed on a rack behind him. As he worked, the rack tipped over and struck him. Employee #1 suffered crushing injuries and fractures in both hips. Employee #1 was transported to a medical center, where he received treatment and was then hospitalized.
