Investigation Summary
At approximately 11:40 a.m. on June 15, 2017, Employee #1, #2 and a crew were excavating a pipeline in preparation for site work. As they worked, the trench collapse. The trench wall fell in and upon Employee #1 causing crushing injuries to him. Employee #2 experienced minor injuries in the trench collapse. Employee #1 was transported to a medical center, where he received treatment and was then hospitalized.
