Investigation Summary
On or about August 12, 1994, Employee #1 and a coworker were on a loading dock unloading crates of live chickens from a flatbed trailer and placing them on the dock platform. The dock platform was about 48 ft above a concrete drive-in bay. A coworker was removing the chickens from the crates and then stacking the empty crates back on the truck. Employee #1 had been working for about 1 1/2 hours when he fell from the dock platform to the concrete drive. The coworker went to assist Employee #1 and asked someone to call an ambulance. Employee #1 was moved away from the dock and CPR was performed. An ambulance arrived and removed Employee #1. The coroner's report listed the cause of death as a heart attack.
