Employee dies when caught by automatic loading device

At approximately 7:15 a.m. on or about February 24, 1994, Employee #1 was looking beyond a removed fence guard on the RGV line. The RGV, an automatic load and unload device, automatically started up and caught Employee #1 in the abdominal area between machining max #6 and the RGV, causing fatal blunt force trauma.
