Employee asphyxiates when forklift overturns

Employee #1 was using a lift truck assigned to his department to exit the plant to talk with his supervisor who was outside the plant. After talking with the supervisor, Employee #1 drove off and made a U-turn. The lift truck went into a 13-inch recessed area alongside the driveway and turned over, Employee #1 landed on the ground, and the truck landed on him. The mast struck his abdomen, pushing his organs up against his respiratory system, which led to asphyxiation. The 12-foot wide recessed area went for approximately 14 feet alongside the driveway. There were no barriers or guardrails.
